Seeking a Nurse Practitioner with home health experience to provide high-quality risk assessments for Medicare Advantage plan members in Harnett County, NC. This per diem opportunity offers flexibility and autonomy, with the ability to design your own schedule and work in locations such as Lillington, Anderson Creek, and Spout Springs. Each home health visit involves conducting thorough risk assessments lasting approximately 45 minutes, which may include annual health assessments or post-discharge visits.

Schedule:

• Flexible shifts, available Monday through Sunday
• Self-scheduling to fit your availability
• Opportunity for weekly or bi-weekly hourly commitments

Key Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage plan members
• Assess both prior and current medical history, medical conditions, and medication usage
• Educate clients on managing their medical conditions and understanding medications
• Identify care gaps and recommend next steps to promote health and safety
• Note: No treatment plan development or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in North Carolina
• Board Certification via AANP or ANCC
• MSN from an accredited nursing program
• Current BLS/CPR certification
• Minimum of 6 months current experience as a Nurse Practitioner preferred
• Prior home health experience is a plus

Providers are compensated at $80-$90 per evaluation, with additional mileage and administrative time stipends.
