Job Description

Experienced Nurse Practitioners are invited to play a pivotal role in providing quality home-based healthcare evaluations for Medicare Advantage plan members. This per diem opportunity centers around conducting home health risk assessments in various areas throughout Harnett County, allowing you to shape your schedule and positively impact your community.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments
  • Evaluate patients medical history, current conditions, and medication use
  • Educate patients on medical conditions and medication adherence
  • Identify care gaps and offer actionable recommendations to improve patient outcomes
  • Visits generally last about 45 minutes each, and may take place as annual assessments or post-discharge check-ins
  • No medication prescribing or treatment plan development required

Qualifications:

  • Current Nurse Practitioner license (NC)
  • CPR certification (BLS required)
  • Board certification via AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited program
  • At least 6 months recent NP experience preferred

This position offers compensation of $80-$90 per evaluation, including a mileage stipend and payment for administrative time. Youll work within prominent communities throughout Harnett County, including Lillington, Anderson Creek, and Spout Springs, supporting patients from Dunn and the surrounding areas.
