Step into a pivotal per diem role providing impactful home health assessments for Medicare Advantage plan members throughout Fayette County, Indiana. This position is ideal for Nurse Practitioners seeking flexibility and autonomy, with the ability to design your own schedule and deliver direct care in patients homes across communities such as Connersville, Glenwood, and Bentonville.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduling for optimal work-life balance
- Minimum hourly commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform annual home health risk assessments, typically taking 45 minutes per visit
- Travel to various patient homes within Fayette County for individual assignments
- Review and document comprehensive medical, medication, and surgical histories
- Educate patients regarding their medical conditions and applicable medications
- Identify gaps in preventative care or medication management and suggest solutions
- Focus exclusively on assessmentsno prescribing or treatment plan creation required
Qualifications:
- Valid Indiana Nurse Practitioner License
- Active, unexpired CPR certification (BLS)
- Board certification (AANP or ANCC)
- Master of Science in Nursing (MSN) from an accredited institution
- At least six months of recent NP experience preferred
Compensation for this opportunity ranges from $80$90 per completed evaluation, and includes mileage stipends as well as payment for administrative timereflecting the importance and flexibility of the position.
