This per diem opportunity is ideal for skilled Nurse Practitioners passionate about patient-centered home health assessments. Professionals will conduct comprehensive health risk evaluations for Medicare Advantage members in various locations within Fayette County, Indiana, including towns such as Connersville, Glenwood, and Bentonville. With the flexibility to create your own schedule, youll support at-risk populations by performing thorough annual or post-discharge assessmentsall without the need for treatment planning or prescribing medications.

Schedule:

• Flexible shifts available Monday through Sunday
• Choose your own workdays and hours
• Commit to a minimum number of hours weekly or bi-weekly

Key Responsibilities:

• Perform annual home health risk assessments
• Evaluate patient medical histories, current and past medical conditions, and medication usage
• Educate patients about their health conditions and medications, empowering them with knowledge
• Identify care gaps and recommend strategies to address them
• Focus solely on assessmentno prescribing or treatment plan development required

Qualifications:

• Active Nurse Practitioner license in the state of Indiana
• Current CPR certification (BLS)
• Board certified by AANP or ANCC
• Six months or more of recent Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution

Compensation for this position is $80$90 per completed evaluation, with additional mileage stipend and pay for administrative time.
