Job Description

Advance your nursing career by providing critical in-home healthcare assessments within Adams County, IL. In this per diem opportunity, you will leverage your Nurse Practitioner expertise to conduct comprehensive home health risk assessments for Medicare Advantage plan members. Assignments may include annual health assessments or post-discharge visits, tailored to individuals residing in Quincy, Camp Point, Payson, and surrounding communities. Each visit typically spans about 45 minutes.

  • Commit to a minimum number of weekly or bi-weekly hours to suit your availability

Key Responsibilities:

  • Perform detailed home health risk assessments for Medicare Advantage members
  • Review and evaluate patients prior and current medical histories, conditions, and medication regimens
  • Educate patients on their medical conditions and prescribed medications
  • Identify care gaps and offer recommendations to address them
  • Focus on risk assessmentno treatment plans or medication prescribing required

Qualifications:

  • Valid Nurse Practitioner License in IL
  • Active CPR certification (BLS)
  • National Board Certification via AANP or ANCC
  • MSN degree from an accredited nursing program
  • Minimum six months experience as a Nurse Practitioner preferred
