Take on a meaningful clinical role dedicated to delivering home health risk assessments for Medicare Advantage plan members across Adams County. This per diem Nurse Practitioner assignment offers latitude over your schedule and variety in your day, visiting patients in their homes in areas such as Quincy, Camp Point, and Payson. Each visit averages 45 minutes, ranging from annual health assessments to post-discharge evaluations, without requiring you to prescribe medications or develop treatment plans.

- Commitment can be set on a weekly or bi-weekly basis
Responsibilities:
- Perform comprehensive home health risk assessments for Medicare Advantage participants, assessing both current and previous health history
- Review and document existing medical conditions and medication usage
- Offer education to patients regarding their health conditions and medications
- Detect potential care gaps, providing informed recommendations to patients and their care team
- Complete accurate documentation after each assessment
- Collaborate effectively with interdisciplinary teams as needed
Qualifications:
- Valid Nurse Practitioner license (IL)
- Board certification via AANP or ANCC
- Master of Science in Nursing from an accredited institution
- Active CPR certification (BLS)
- Minimum of 6 months NP experience is preferred
- Reliable transportation to travel within Adams County
This role provides $80$90 per completed evaluation, in addition to a mileage stipend and compensation for administrative time.
