This per diem opportunity allows Nurse Practitioners to deliver quality care directly in patients homes, with assignments across Lexington, Parkers Crossroads, and Sardis. With flexible scheduling between Monday and Sunday and the ability to design your own hours, this role is ideal for clinicians seeking autonomy and work-life balance.

Schedule:

  • Choose flexible shifts any day of the week (MondaySunday)
  • Set your own schedule and commit to minimum weekly or bi-weekly hours that fit your availability

Key Responsibilities:

  • Complete comprehensive home health risk assessments, including annual and post-discharge evaluations
  • Review prior and current medical history, assess ongoing medical conditions, and document medication usage
  • Provide education to members regarding their health conditions and prescribed medications
  • Identify gaps in care and deliver actionable recommendations to support improved patient outcomes
  • Note: No development of treatment plans or medication prescribing is needed

Qualifications:

  • Active Nurse Practitioner license in the state of TN
  • Board Certification via AANP or ANCC required
  • Master of Science in Nursing (MSN) from an accredited program
  • Current BLS/CPR certification
  • A minimum of six months of recent Nurse Practitioner experience preferred
