This per diem position offers the flexibility to create your own schedule, with available shifts seven days a week. Schedule:

Key Responsibilities:
- Conduct comprehensive home health risk assessments, typically lasting 45 minutes, in Lexington and neighboring communities such as Parker's Crossroads and Sardis
- Review and evaluate patients prior and current medical histories, health conditions, and medication usage
- Deliver patient education on management of chronic conditions and medication adherence
- Identify and communicate care gaps, recommending solutions to improve outcomes
- Focus solely on assessmentno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Current CPR (BLS) certification
- Board Certification through AANP or ANCC
- Minimum of 6 months NP experience preferred
- MSN degree from an accredited nursing program
