Experience an engaging opportunity as a Nurse Practitioner specializing in home health risk assessments on a per diem basis. As part of this dynamic role, youll conduct thorough, in-person risk assessments for Medicare Advantage plan members across a variety of communities within Pike County, ARincluding Glenwood, Murfreesboro, and Delight. Each assessment typically takes about 45 minutes and may involve annual health reviews or post-discharge visits to ensure members receive the highest quality of preventative care.
Schedule:
- Flexible shifts available any day between Monday and Sunday

- Minimum weekly or bi-weekly hourly commitments to fit your lifestyle
Responsibilities:
- Perform comprehensive annual risk assessments for Medicare Advantage members in their homes
- Carefully review patients prior and current medical history, health conditions, and medication use
- Offer clear education on diagnosed conditions and prescribed medications to enhance patient understanding
- Pinpoint any gaps in care and provide valuable recommendationsno prescribing or treatment plan creation required
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current CPR certification (BLS required)
- Board certification through AANP or ANCC
- At least 6 months NP experience preferred
- MSN degree from an accredited nursing program
