Schedule:

• Flexible shifts available seven days a week
• Set your own hours
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive home health risk assessments for Medicare Advantage members within Lee County
• Perform evaluations that take approximately 45 minutes, including annual health visits and post-discharge assessments
• Assess prior and current medical history, chronic conditions, and medication regimens
• Provide patient education on ongoing health issues and prescribed medications
• Identify gaps in care and offer actionable recommendations; no treatment planning or prescription responsibilities

Required Qualifications:

• Current Nurse Practitioner licensure in Arkansas
• Board Certification from AANP or ANCC
• MSN degree from an accredited institution
• Active CPR (BLS) certification
• Minimum of six months recent NP experience (preferred)
