Job Description

A rewarding per diem opportunity is available for Nurse Practitioners experienced in home health. This role centers around providing comprehensive, in-home health risk assessments for Medicare Advantage plan members across Nez Perce County.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Self-scheduling allows you to tailor shifts to your availability
  • Minimum hourly commitment weekly or bi-weekly

Primary Duties:

  • Conduct in-person annual home health risk assessments with patients in their homes
  • Assess and review previous and current medical history, including medication use and diagnoses
  • Deliver education to members regarding their health conditions and prescribed medications
  • Detect gaps in care and issue recommendations to improve patient outcomes
  • No responsibility for creating treatment plans or prescribing medications

Desired Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Board Certification through AANP or ANCC
  • Masters degree in Nursing (MSN) from an accredited institution
  • Current BLS certification
  • At least six months of recent Nurse Practitioner experience (preferred)
  • Strong interpersonal and assessment skills

This position compensates $80$90 per evaluation, in addition to a mileage stipend and recognition for administrative time. You can expect diverse settings, as shifts may include visits to patients throughout cities and communities like Lewiston, Lapwai, and Peck within Nez Perce County. The average visit lasts approximately 45 minutes, and assignments may include both annual assessments and post-discharge visits.
