This per diem opportunity provides the flexibility to create your own schedule and commit to shifts that fit your lifestyle.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own schedule
• Options for minimum weekly or bi-weekly hourly commitments

Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage plan members during in-home visits, lasting approximately 45 minutes each
• Assess prior and current medical history, medical conditions, and medication use
• Deliver patient education regarding medical conditions and prescribed medications
• Identify gaps in patient care and make recommendations to support overall health
• Focus exclusively on assessmentsno care plan creation or medication prescribing required

Qualifications:

• Valid Nurse Practitioner license in Tennessee
• Current CPR certification (BLS)
• Board Certification from AANP or ANCC
• At least 6 months of recent nurse practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited institution

Nurse Practitioners in this per diem role receive $80$90 per evaluation completed, with additional compensation for mileage and administrative duties.
