Advance your career as a Nurse Practitioner specializing in home health risk assessments with a leading Medicare Advantage health plan serving patients throughout Maury County, TN. This per diem opportunity allows you to design a highly flexible schedule that fits your lifestyle, selecting shifts and locations in Columbia, Spring Hill, Mount Pleasant, and the surrounding communities.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform comprehensive annual and post-discharge risk assessments in members homes
• Assess prior and current medical history, conditions, and medications
• Educate patients on health conditions and medication regimens
• Identify and address gaps in care with actionable recommendations
• No treatment plan development or medication prescribing required

Qualifications:

• Active Nurse Practitioner license in Tennessee
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• Master of Science in Nursing (MSN) from an accredited program
• At least 6 months of recent Nurse Practitioner experience preferred
