Bring your advanced practice expertise to a dynamic per diem opportunity, where you will conduct home health risk assessments for Medicare Advantage members across Burke County, North Carolina. This role allows you to make a meaningful impact by supporting preventive healthcare and wellness for patients in their homes. As a Nurse Practitioner conducting these assessments, youll enjoy flexibility, autonomy, and the chance to work with diverse patient populations.
Schedule:
- Flexible shifts Monday through Sunday
- Create your own work schedule
- Choose a minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual health risk assessments in member homes across Burke County (including Morganton, Hickory, Valdese, and surrounding areas)
- Evaluate and document prior and current medical history, conditions, and medication usage
- Educate patients on managing chronic conditions and medications as appropriate
- Identify gaps in care and communicate recommendations for follow-up or further evaluation
- No need to develop treatment plans or prescribe medications
Preferred Skills & Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing school
- At least 6 months of Nurse Practitioner experience (strongly preferred)
- Excellent communication, assessment, and documentation skills
This role offers competitive pay of $80-$90 per completed evaluation, with an additional mileage stipend and compensated administrative time.
