Take on a vital role in delivering personalized care as a Nurse Practitioner dedicated to home health risk assessments in Burke County, NC, including areas such as Hickory, Morganton, and Valdese. This per diem position offers exceptional flexibilityshifts are available Monday through Sunday, and you'll have the autonomy to build your own schedule, with both weekly and bi-weekly minimum hourly commitments.

Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage members in their homes.
- Review and document prior and current medical history, ongoing health conditions, and current medications.
- Identify care gaps and recommend solutionsno ongoing treatment planning or prescription authority required.
- Educate patients about their medical conditions and prescribed medications, empowering them to manage their health.
Qualifications:
- Active Nurse Practitioner license in North Carolina.
- Current BLS certification (CPR).
- Board Certification by AANP or ANCC.
- MSN from an accredited nursing program.
- At least 6 months of Nurse Practitioner experience preferred.
Enjoy compensation between $80$90 per evaluation, with additional stipends for mileage and administrative time.
