Job Description

Experience a rewarding per diem opportunity as a skilled Nurse Practitioner delivering comprehensive home health risk assessments. This position centers on providing compassionate, patient-centered care to Medicare Advantage plan members, with each assessment typically requiring about 45 minutes. Assignments vary and may consist of conducting annual evaluations or visiting patients post-discharge in Blount County, TNincluding Maryville, Alcoa, and Friendsville.

Schedule:

  • Flexible shifts available seven days a week
  • Set your own schedule to suit your needs
  • Minimal required commitment available weekly or bi-weekly

Key Responsibilities:

  • Perform in-home annual risk assessments for Medicare Advantage health plan members
  • Review and evaluate patients prior and current medical history, conditions, and medications
  • Deliver straightforward education on diagnoses, health conditions, and medications
  • Identify care gaps and provide actionable recommendations for follow-up or intervention
  • Focus exclusively on assessmentno prescribing or development of treatment plans required

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current CPR certification (BLS)
  • Board certified via AANP or ANCC
  • Minimum of 6 months recent experience as a Nurse Practitioner is preferred
  • MSN degree from an accredited nursing program
