Job Description

Experience a meaningful per diem role providing home-based healthcare in Blount County and surrounding areas. This opportunity is perfect for Nurse Practitioners seeking flexibility while making a direct impact on patient wellness through home health risk assessments. Youll work with Medicare Advantage members, assisting with annual health evaluations or post-discharge visits in locations such as Maryville, Alcoa, and Friendsville. Each assessment averages 45 minutes, giving you the chance to deliver quality care with a focus on patient education and care gap identification.

Schedule:

  • Flexible shifts available any day of the week, Monday through Sunday
  • Set your own schedule for optimal work-life balance
  • Choose your minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage members
  • Review and evaluate patients past and current medical history, existing conditions, and medication lists
  • Educate patients on their health conditions, treatments, and medication regimens
  • Identify care gaps and recommend steps for improvement
  • No requirement to prescribe medication or devise treatment plans

Qualifications:

  • Active Nurse Practitioner license in Tennessee
  • Current CPR certification (BLS required)
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • At least 6 months current NP work experience preferred

Enjoy compensation of $80$90 for each completed evaluation, with additional stipends for mileage and administrative time.
