Job Description
Take on a rewarding opportunity as a Nurse Practitioner offering home health risk assessments for Medicare Advantage plan members. This per diem position focuses on delivering care within Madison County, NC, including communities such as Mars Hill, Marshall, and Hot Springs. Assessments will be home-based, supporting patients with annual health reviews or post-discharge evaluations. Each visit is expected to last approximately 45 minutes, with the location and nature of each appointment varying according to patient needs.

- Commit to a minimum weekly or bi-weekly number of hours
Key Responsibilities:
- Conduct home health risk assessments for Medicare Advantage members
- Evaluate prior and current medical history, conditions, and medications
- Educate patients regarding ongoing medical conditions and prescribed medications
- Identify care gaps and provide recommendations to address them
- No responsibility for treatment plans or medication prescribing
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- Minimum of 6 months recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This role provides a competitive rate of $80-$90 per evaluation, along with a mileage stipend and compensation for administrative time.
