Job Description
Provide essential care and support to patients in their homes by performing comprehensive risk assessments as a Nurse Practitioner specializing in home health. In this vital per diem opportunity, youll make a meaningful impact on Medicare Advantage members across Lake County, including Clermont, Leesburg, and Eustis. Each assessment takes approximately 45 minutes and may focus on annual health reviews or post-discharge visits. Candidates may be scheduled in any of the countys locations during individual shifts.

- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct thorough annual home health risk assessments for Medicare Advantage plan members
- Evaluate and document patients prior and current medical histories, conditions, and medication usage
- Educate patients on their health conditions and prescribed medications
- Identify any gaps in care and make recommendations for improving patient outcomes
- Provide clear communication and high-quality patient experience
- Note: No treatment plans or prescription responsibilities are required
Qualifications:
- Active Nurse Practitioner License in FL
- Current BLS (CPR) certification
- Board Certification via AANP or ANCC
- Minimum of 6 months of current Nurse Practitioner experience preferred
- Graduate of an accredited MSN program
This role provides compensation of $80-$90 per evaluation, with an additional stipend for mileage and administrative time, recognizing the value of your specialized care.
