Enjoy a flexible per diem opportunity as a Nurse Practitioner focusing on home health risk assessments within Sumter County, FL. This role allows you to set your own schedule and support Medicare Advantage members who reside in areas such as The Villages, Wildwood, and Bushnell. Each visit typically lasts around 45 minutes and may encompass annual health assessments or post-discharge evaluations to improve overall patient outcomes.
Schedule:
- Flexible shifts available Monday through Sunday

- Commit to a minimum weekly or bi-weekly hourly requirement
Key Responsibilities:
- Conduct comprehensive home health risk assessments for Medicare Advantage plan members
- Review and evaluate patients' medical histories, current conditions, and medications
- Deliver clear, compassionate education regarding health conditions and medication management
- Identify care gaps, recommend appropriate next steps, and document findings accurately
- No need for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Florida
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification
- Master of Science in Nursing (MSN) from an accredited institution
- Preferred: Minimum of six months recent experience as an NP, ideally in a home health setting
