In this per diem role, you will provide essential healthcare services to Medicare Advantage plan members within Linn County, including Lebanon, Sweet Home, and Albany, OR. Key Responsibilities:

• Conduct annual home health risk assessments for Medicare Advantage members
• Review and evaluate prior and current medical history, medical conditions, and a complete medication history
• Educate patients on their medical conditions and medications
• Identify care gaps and provide recommendations to address them
• Visits vary (annual health assessment or post-discharge), generally lasting about 45 minutes each
• No requirement for treatment planning or prescription writing

Qualifications:

• Active Nurse Practitioner license in Oregon
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• Minimum of six months of NP experience preferred
• MSN degree from an accredited nursing program

Earn $80-$90 per completed evaluation, with additional mileage stipend and compensation for administrative time.
