Job Description

Experience meaningful impact as a Nurse Practitioner delivering vital home health risk assessments for Medicare Advantage health plan members in Boone County and surrounding communities. This flexible per diem opportunity is ideal for motivated professionals who enjoy building relationships in a home-based setting while helping improve patient outcomes.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual and post-discharge home health risk assessments for Medicare Advantage member visits (typically 45 minutes per visit)
  • Evaluate patients prior and current medical histories, health conditions, and medication use
  • Educate individuals and families on current medical conditions and medication management
  • Identify gaps in care and make recommendations to optimize patient well-being
  • No requirement to create treatment plans or prescribe medications

Required Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Master of Science in Nursing (MSN) from an accredited institution
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • At least 6 months of recent Nurse Practitioner experience preferred
