Key Responsibilities:

• Perform annual home health risk assessments for Medicare Advantage plan members in patient homes
• Evaluate patients prior and current medical history, medical conditions, and medication usage
• Provide valuable education to patients regarding their health issues and medications
• Identify care gaps and share recommendations; no development of treatment plans or prescription duties required

Qualifications:

• Active Nurse Practitioner license in Indiana
• Current CPR (BLS) certification
• Board Certification through AANP or ANCC
• Minimum of 6 months current Nurse Practitioner experience preferred
• Master of Science in Nursing (MSN) from an accredited program
