Job Description

This role involves conducting home health risk assessments within Dearborn County, Indianaincluding Lawrenceburg, Greendale, and Auroraserving Medicare Advantage members. Work is scheduled per your availability, empowering you to manage your own caseload and coordinate visits that fit your lifestyle.

Schedule:

  • Flexible shifts available any day Monday through Sunday
  • Set your preferred hours and frequency
  • Minimum weekly or bi-weekly hour commitment to suit your needs

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical conditions, medical history, and medication usage
  • Provide patient education regarding chronic conditions and medications
  • Identify care gaps and recommend appropriate interventionsno prescribing or independent treatment planning required
  • Assessments last approximately 45 minutes, covering general wellness or post-discharge needs

Ideal Qualifications:

  • Active Nurse Practitioner license (IN)
  • Board Certification (AANP or ANCC)
  • Masters degree in Nursing (MSN) from an accredited institution
  • Current BLS/CPR certification
  • At least 6 months of recent nurse practitioner experience preferred
