Make a meaningful impact in patient care by conducting home health risk assessments for Medicare Advantage members in Dearborn County, IN. This per diem opportunity offers the flexibility to design your own schedule, with options for shifts throughout the week to best suit your lifestyle and commitments.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own hours and frequency of work
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments in patients homes, covering areas such as Lawrenceburg, Greendale, and Aurora
- Review patients medical histories, current conditions, and medication usage
- Educate patients on their health conditions and medications to foster a better understanding of their care
- Identify potential gaps in care and provide recommendations to address these issues
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current BLS (CPR) certification
- Board Certification via AANP or ANCC
- Master of Science in Nursing from an accredited institution
- A minimum of six months of recent Nurse Practitioner experience is preferred
