Deliver compassionate, high-quality care by performing home health risk assessments for Medicare Advantage plan members in Rutherford County, NC. This dynamic per diem opportunity empowers Nurse Practitioners to make a positive impact within the community while enjoying schedule flexibility and professional autonomy.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Conduct annual in-home risk assessments for Medicare Advantage members
- Assess medical history, current medical conditions, and medication regimens during visits
- Educate patients on chronic conditions, medication management, and preventive health
- Identify gaps in care and share recommendations to address and resolve them
- Complete thorough documentation; no treatment plan creation or prescribing required
Qualifications:
- Active Nurse Practitioner license in North Carolina
- Board Certification through AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited program
- Current, valid CPR (BLS) certification
- A minimum of 6 months recent Nurse Practitioner experience preferred
This role offers per evaluation compensation ranging from $80-$90, in addition to mileage stipends and administrative time, reflecting your expertise and effort.
