Make a meaningful impact by providing comprehensive home health risk assessments to Medicare Advantage plan members throughout Crawford County, AR. This per diem opportunity offers significant autonomy, flexibility in scheduling, and a chance to connect directly with the community in Van Buren, Alma, Mulberry, and beyond.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Design your own schedule for better work-life balance
- Commit to either a weekly or bi-weekly minimum hour requirement
Key Responsibilities:
- Complete annual home health risk assessments, spending about 45 minutes per evaluation
- Review and document patients current and prior medical history, including medication use and existing medical conditions
- Educate patients about their diagnoses and medications, supporting informed, empowered health management
- Identify care gaps and recommend solutions to optimize overall patient wellbeing
- Focus solely on risk assessmentno need for creating treatment plans or prescribing medication
Qualifications:
- Active Nurse Practitioner License in Arkansas
- Board Certification via AANP or ANCC
- Current CPR (BLS) certification required
- MSN from an accredited nursing program
- At least 6 months of recent NP experience preferred
