Deliver expert care in the community by performing comprehensive home health risk assessments for Medicare Advantage plan members throughout Crawford County, AR. As a Nurse Practitioner specializing in home health, youll be instrumental in identifying patient needs and empowering healthier livesall while enjoying the autonomy and flexibility that fits your schedule.
Schedule:
- Flexible shifts available between Monday and Sunday
- Self-scheduling to fit your preferred availability
- Minimum weekly or bi-weekly hourly commitments
Key Responsibilities:
- Conduct thorough annual and post-discharge home health risk assessments in patients residences across Van Buren, Alma, Mulberry, and surrounding areas
- Review and evaluate medical and medication history, noting current and previous health conditions
- Educate members regarding chronic conditions, acute issues, and medication usage
- Identify any gaps in care and provide practical, actionable recommendations
- No requirement to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- Minimum of 6 months of recent experience as a Nurse Practitioner preferred
- Master of Science in Nursing (MSN) from an accredited program
