Job Description

Deliver specialized care where it matters mostdirectly in the community. This per diem role offers Nurse Practitioners the opportunity to conduct in-home health risk assessments for Medicare Advantage members in Laurinburg and across Scotland County, NC. Youll have the flexibility to build your own schedule, selecting shifts between Monday and Sunday according to your availability. A minimum weekly or bi-weekly hourly commitment is required, ensuring a balanced workload that fits your life.

Responsibilities:

  • Perform comprehensive home health risk assessments, each averaging 45 minutes, for Medicare Advantage members
  • Visit patients in Laurinburg, Wagram, Gibson, and throughout Scotland County, NC
  • Review and evaluate current and prior medical history, ongoing medical conditions, and medication lists
  • Educate patients about their medical conditions and prescribed treatments
  • Identify care gaps and recommend steps to improve member health; no need to create treatment plans or prescribe medications
  • Document assessments thoroughly and submit timely reports

Qualifications:

  • Active, unrestricted Nurse Practitioner license in North Carolina
  • Current BLS (CPR) certification
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing school
  • At least 6 months experience as a practicing NP preferred
