Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform home health risk assessments on Medicare Advantage plan members in Sussex County, including Milford, Seaford, and Georgetown
- Complete evaluations within clients homesvisits typically last around 45 minutes
- Assess both current and previous medical history, ongoing medical conditions, and medication use
- Educate members regarding medical conditions and medications as needed
- Identify gaps in care and offer actionable recommendations
- No responsibility for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Delaware
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN from an accredited nursing school
- A minimum of 6 months of nurse practitioner experience preferred
