Schedule:

• Flexible shifts available Monday through Sunday
• Create your own work schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform home health risk assessments on Medicare Advantage plan members in Sussex County, including Milford, Seaford, and Georgetown
• Complete evaluations within clients homesvisits typically last around 45 minutes
• Assess both current and previous medical history, ongoing medical conditions, and medication use
• Educate members regarding medical conditions and medications as needed
• Identify gaps in care and offer actionable recommendations
• No responsibility for creating treatment plans or prescribing medications

Qualifications:

• Active Nurse Practitioner License in Delaware
• Board Certification through AANP or ANCC
• Current CPR certification (BLS)
• MSN from an accredited nursing school
• A minimum of 6 months of nurse practitioner experience preferred
