Deliver comprehensive home health risk assessments as a Nurse Practitioner within San Miguel County, NM. You will visit Medicare Advantage plan members in their homes, including areas such as Las Vegas, Pecos, and Montezuma. Each assessment is typically completed in 45 minutes and can range from annual health reviews to post-discharge evaluations. This per diem position offers exceptional flexibility and autonomy, empowering you to manage your schedule while making a direct impact on patient well-being.

Schedule:

• Flexible shifts available throughout the week (Monday through Sunday)
• Opportunity to create your own schedule
• Minimum commitment on a weekly or bi-weekly basis

Core Responsibilities:

• Perform comprehensive home health risk assessments for Medicare Advantage members
• Review and evaluate each patients current and prior medical history, conditions, and medication lists
• Educate patients about their health conditions, treatment guidelines, and medications
• Identify gaps in care and recommend follow-up action as needed
• Focus strictly on assessmentno prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner license in New Mexico
• Board Certification via AANP or ANCC
• Valid CPR certification (BLS)
• Master of Science in Nursing (MSN) from an accredited institution
• At least six months of recent NP experience is preferred
