Healthcare professionals with expertise in home health assessments are invited to take on a dynamic per diem opportunity, offering service flexibility and professional autonomy. As a Nurse Practitioner serving Medicare Advantage plan members, you will conduct thorough home health risk assessments in patient residences across various areas of Pinellas County.
Schedule:
- Flexible shifts available Monday through Sunday
- Set your own schedule for optimal work/life balance
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments on Medicare Advantage members
- Collect and review prior/current medical history, assess ongoing medical conditions, and document comprehensive medication histories
- Provide patient and family education related to chronic conditions and medication management
- Identify any gaps in care, offering recommendations to support optimal patient health
- No treatment planning or medication prescribing required during visits
Qualifications:
- Active and unrestricted Nurse Practitioner license in Florida
- Current CPR/BLS certification
- Board Certification through AANP or ANCC
- At least 6 months of recent experience as a Nurse Practitioner (preferred)
- MSN from an accredited nursing program
This per diem role provides an attractive compensation package, offering $80-$90 per completed evaluation, as well as mileage reimbursement and administrative time, making it well-suited for independent, motivated practitioners.
