Job Description

An exciting opportunity is available for experienced Nurse Practitioners to deliver home health risk assessments on a per diem basis within Nez Perce County, ID. This role offers maximum flexibility and the chance to make a meaningful impact in your community. Assessments typically last 45 minutes and may range from annual health check-ups to post-discharge visits for Medicare Advantage plan members residing in areas such as Lewiston, Lapwai, and Peck.

Schedule:

  • Flexible shifts available between Monday and Sunday
  • Set your own schedule
  • Choose a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate comprehensive health histories and current conditions
  • Assess medication usage and provide education to patients
  • Identify gaps in care, offering actionable recommendations for improvement
  • Educate patients on medical conditions, medications, and preventative measures
  • No treatment plan development or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Idaho
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN degree from a nationally accredited nursing program
  • At least 6 months of current experience as a Nurse Practitioner is preferred

Compensation: Earn $80-$90 per evaluation, plus mileage stipend and administrative time.
