Take on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health within Nez Perce County, conducting comprehensive risk assessments for Medicare Advantage plan members. Youll be entrusted with delivering thorough home evaluations for individuals in locations such as Lewiston, Lapwai, and Peck, ensuring their health needs are proactively managed. Each assessment typically lasts around 45 minutes, ranging from annual health reviews to post-discharge visits.
Schedule:
- Flexible shifts available between Monday and Sunday
- Create your own schedule to best suit your availability
- Commitment to a minimum number of hours per week or every two weeks
Qualifications:
- Active Nurse Practitioner license in Idaho
- Board Certification through AANP or ANCC required
- Current CPR (BLS) certification
- MSN degree from an accredited nursing school
- At least 6 months of current nurse practitioner experience preferred
Key Responsibilities:
- Perform annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate patients current and past medical histories, medical conditions, and medication regimens
- Educate patients on their medical conditions and prescribed medications as appropriate
- Identify care gaps and provide actionable recommendations to help resolve them
- Role strictly focused on assessmentsno creation of treatment plans or medication prescribing required
