Deliver vital healthcare insights as a per diem Nurse Practitioner serving Medicare Advantage plan members through home health risk assessments. In this dynamic role, you'll visit individuals in their homes throughout Lancaster County, providing valuable evaluations with a flexible, self-directed schedule.

Schedule:

• Flexible shifts available Monday through Sunday
• Design your own working hours
• Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

• Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
• Assess prior and current medical history, chronic conditions, and medication use
• Offer patient education on health conditions and prescription regimens
• Identify care gaps and recommend appropriate steps for improved outcomes
• No responsibility for developing treatment plans or prescribing medication

Qualifications:

• Active Nurse Practitioner license for South Carolina
• Board Certification from AANP or ANCC
• MSN degree from an accredited nursing program
• Current CPR (BLS) certification
• Prefer at least six months of recent nurse practitioner experience
