Advance your Nurse Practitioner career by delivering high-quality care in a flexible, per diem capacity. This opportunity focuses on conducting in-home health risk assessments for Medicare Advantage plan members located throughout Wyoming County, including Attica, Warsaw, and Perry. You will provide valuable insights into members health by performing comprehensive assessmentswithout the responsibility for treatment planning or prescription management.
Schedule:
- Flexible scheduling Monday through Sunday
- Self-directed schedule creation
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments tailored to Medicare Advantage members
- Evaluate prior and current medical history, ongoing medical conditions, and medication usage during 45-minute visits
- Deliver patient education regarding medical conditions and medications in the home setting
- Identify and address gaps in care, offering recommendations to enhance care outcomes
- Visits may also include post-discharge assessments as needed
Qualifications:
- Active Nurse Practitioner license in New York
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- MSN Degree from an accredited program
- At least 6 months of recent Nurse Practitioner experience is highly preferred
This per diem position allows you to create your own schedule, ensuring a healthy work-life balance. In addition to a competitive rate of $80$90 for each completed evaluation, you will also receive a mileage stipend and administrative time compensation.
