Play a pivotal role in improving community health by performing home health risk assessments for Medicare Advantage plan members across Pope County, AR. As a skilled Nurse Practitioner specializing in home health, you will conduct comprehensive evaluations with the flexibility to manage your own schedule and contribute meaningfully to patient care.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule to fit your availability
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Carry out annual home health risk assessments for Medicare Advantage members in various communities throughout Pope County, including Russellville, Pottsville, and Atkins
- Review medical history, evaluate current conditions, and assess medication use
- Offer patient education on managing existing conditions and medication adherence
- Recognize and document care gaps, offering actionable recommendations to enhance health outcomes
- Visits typically last about 45 minutes and may include annual assessments or post-discharge evaluations
- Note: No responsibility for managing treatment plans or prescribing medication
Qualifications:
- Active Nurse Practitioner license in Arkansas
- Current BLS/CPR certification
- Board certification from either AANP or ANCC
- Accredited MSN degree in nursing
- Minimum of 6 months experience as a Nurse Practitioner is preferred
This per diem opportunity offers compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative time, providing both flexibility and competitive earnings for dedicated professionals.
