Job Description

Play a pivotal role in improving community health by performing home health risk assessments for Medicare Advantage plan members across Pope County, AR. As a skilled Nurse Practitioner specializing in home health, you will conduct comprehensive evaluations with the flexibility to manage your own schedule and contribute meaningfully to patient care.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule to fit your availability
  • Minimum weekly or bi-weekly hourly commitment required

Responsibilities:

  • Carry out annual home health risk assessments for Medicare Advantage members in various communities throughout Pope County, including Russellville, Pottsville, and Atkins
  • Review medical history, evaluate current conditions, and assess medication use
  • Offer patient education on managing existing conditions and medication adherence
  • Recognize and document care gaps, offering actionable recommendations to enhance health outcomes
  • Visits typically last about 45 minutes and may include annual assessments or post-discharge evaluations
  • Note: No responsibility for managing treatment plans or prescribing medication

Qualifications:

  • Active Nurse Practitioner license in Arkansas
  • Current BLS/CPR certification
  • Board certification from either AANP or ANCC
  • Accredited MSN degree in nursing
  • Minimum of 6 months experience as a Nurse Practitioner is preferred

This per diem opportunity offers compensation of $80$90 per evaluation, along with a mileage stipend and payment for administrative time, providing both flexibility and competitive earnings for dedicated professionals.
