Experienced nurse practitioners are invited to perform impactful home health risk assessments within Thurston County, WAincluding Lacey, Olympia, and Tumwater. This per diem opportunity is ideal for professionals who thrive in independent environments and appreciate the flexibility of managing their own schedules. Visits may range from annual health evaluations to post-discharge assessments for Medicare Advantage plan members.

Schedule:

• Flexible shifts available any day, Monday through Sunday
• Create your own schedule to fit your lifestyle
• Minimal required commitment per week or bi-weekly, offering maximum autonomy

Responsibilities:

• Conduct in-home risk assessments for Medicare Advantage members
• Review and evaluate medical histories, current health conditions, and medication regimens
• Provide patient education regarding medical conditions, treatment adherence, and medications
• Identify care gaps and recommend appropriate next steps to optimize health outcomes
• Visits average approximately 45 minutes; no medication prescribing or treatment plan development required

Qualifications:

• Active Nurse Practitioner license in the state of Washington
• Board Certification through AANP or ANCC
• Current CPR (BLS) certification
• Masters degree in Nursing (MSN) from an accredited school
• At least 6 months of recent NP experience (preferred)
