This role focuses on conducting home health risk assessments for Medicare Advantage members throughout Chautauqua County, providing valuable support across areas such as Jamestown, Dunkirk, Chautauqua, and Mayville.
Schedule:
- Flexible shifts available any day of the week

- Minimum weekly or bi-weekly hourly commitment
Essential Responsibilities:
- Complete annual risk assessments in patients homes, with each visit typically lasting around 45 minutes
- Assess previous and current medical history, medical conditions, and medication use
- Educate patients on their health conditions and appropriate medication management
- Identify care gaps and make actionable recommendations to address those gaps
- No requirement for treatment plan creation or medication prescribing
Ideal Qualifications:
- Current New York Nurse Practitioner license
- Active Board Certification via AANP or ANCC
- MSN degree from an accredited nursing program
- Valid CPR certification (BLS)
- At least 6 months of recent Nurse Practitioner experience is preferred
This position offers competitive per-evaluation compensation, with $80$90 per completed assessment, as well as mileage stipends and compensation for administrative time.
