Job Description

Take your expertise as a Nurse Practitioner into the community with a rewarding per diem opportunity focused on home health risk assessments. This independent, flexible role involves meeting Medicare Advantage plan members throughout Chautauqua County, providing essential annual and post-discharge health assessments. Visits last approximately 45 minutes and are scheduled in areas including Jamestown, Dunkirk, Chautauqua, Mayville, and other surrounding locations, based on your availability.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Choose your own schedule
  • Minimum weekly or bi-weekly shift commitment required

Key Responsibilities:

  • Conduct thorough annual and post-discharge home health risk assessments for Medicare Advantage plan participants
  • Review prior and current medical histories, ongoing medical conditions, and medication lists
  • Educate patients regarding their medical conditions and prescribed medications
  • Identify gaps in ongoing care and provide actionable recommendations
  • No development of treatment plans or prescribing responsibilities required

Qualifications:

  • Valid Nurse Practitioner license in New York
  • Board Certification from AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current CPR certification (BLS)
  • At least 6 months of recent experience as an NP preferred

For each completed evaluation, compensation ranges from $80 to $90, complemented by a mileage stipend and appropriate administrative time.
