Job Description

This role focuses on conducting home health risk assessments for Medicare Advantage members throughout Chautauqua County, providing valuable support across areas such as Jamestown, Dunkirk, Chautauqua, and Mayville.

Schedule:

  • Flexible shifts available any day of the week

  • Minimum weekly or bi-weekly hourly commitment

Essential Responsibilities:

  • Complete annual risk assessments in patients homes, with each visit typically lasting around 45 minutes
  • Assess previous and current medical history, medical conditions, and medication use
  • Educate patients on their health conditions and appropriate medication management
  • Identify care gaps and make actionable recommendations to address those gaps
  • No requirement for treatment plan creation or medication prescribing

Ideal Qualifications:

  • Current New York Nurse Practitioner license
  • Active Board Certification via AANP or ANCC
  • MSN degree from an accredited nursing program
  • Valid CPR certification (BLS)
  • At least 6 months of recent Nurse Practitioner experience is preferred

This position offers competitive per-evaluation compensation, with $80$90 per completed assessment, as well as mileage stipends and compensation for administrative time.
