As a Nurse Practitioner in this rewarding per diem position, you will visit patients in Lakewood and throughout Ocean County, ensuring they receive the comprehensive evaluations and attention they needall while enjoying the flexibility to design your own schedule.

Schedule:

• Flexible shifts available seven days a week
• Personalized schedulingwork when it fits your life
• Minimum weekly or bi-weekly hour commitments

Responsibilities:

• Perform home health risk assessments, typically about 45 minutes per visit, including annual evaluations or post-discharge check-ins
• Assess prior and current medical histories, conditions, and medication lists
• Educate patients on ongoing medical conditions and prescribed medications
• Identify potential gaps in care and recommend ways to address them
• No need for treatment plan creation or prescription writing

Qualifications:

• Active Nurse Practitioner license in New Jersey
• Board Certification through AANP or ANCC
• Masters degree in Nursing (MSN) from an accredited program
• Current CPR (BLS) certification
• At least six months of recent NP experience preferred, but highly motivated new graduates are welcomed
