This per diem opportunity offers the flexibility to build your own schedule, with shifts available any day of the week. Youll work within Ocean County, conducting visits across locations such as Lakewood, Toms River, and Brick, performing thorough risk assessments for Medicare Advantage plan members.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Set your own minimum commitment: weekly or bi-weekly
Key Responsibilities:
- Conduct in-home annual health risk assessments for Medicare Advantage members
- Evaluate prior and current medical histories, medical conditions, and medication usage
- Educate members on their health conditions and prescribed medications
- Recognize gaps in care and offer appropriate recommendations for follow-up or intervention
- Focus exclusively on assessmentno treatment planning or prescribing required
Qualifications:
- Active Nurse Practitioner license in New Jersey (NJ)
- Board Certification via AANP or ANCC
- MSN degree from an accredited school of nursing
- Current CPR (BLS) certification
- At least 6 months of recent NP experience preferred
