Experienced healthcare professionals seeking per diem opportunities will find this position a compelling fit. This role involves conducting home health risk assessments for Medicare Advantage plan members residing in Lakewood and the greater Ocean County areaincluding Toms River and Brick. The primary responsibility is to perform comprehensive assessments in patients residences, ensuring thorough evaluations and patient education.
Schedule:
- Flexible scheduling options available Monday through Sunday
- Design your own work schedule
- Minimum commitment of agreed-upon hours per week or bi-weekly
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members in designated areas
- Review and evaluate patients prior and current medical history, ongoing medical conditions, and medication regimens
- Educate patients regarding their health conditions and medication usage
- Identify care gaps and suggest solutions for improved patient outcomes
- No requirement to create treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Board Certification via AANP or ANCC required
- Current CPR certification (BLS)
- Minimum of six months current experience as a Nurse Practitioner preferred
- MSN degree awarded by an accredited nursing school
This position offers compensation of $80-$90 per evaluation, and includes a mileage stipend as well as pay for administrative tasks associated with the role.
