Step into a vital Nurse Practitioner role focused on delivering comprehensive home health risk assessments to Medicare Advantage members throughout Morris County, NJ. This per diem opportunity offers exceptional flexibility, allowing you to create your own schedule for optimal work-life balance. Shifts are available any day of the weekMonday through Sundayand can be tailored to meet your weekly or bi-weekly hour commitments.
Key Responsibilities:
- Perform thorough annual home health risk assessments in patients homes across Morris County, including Parsippany-Troy Hills, Mount Olive, and Randolph
- Review and evaluate patients past and current medical history, medical conditions, and medication use
- Provide clear education to patients about their health status and prescribed medications
- Identify any gaps in ongoing care and recommend suggested next steps for improved health outcomes
- Focus solely on assessments and patient educationno treatment planning or prescribing necessary
Qualifications:
- Active Nurse Practitioner license in New Jersey
- Current CPR (BLS) certification
- Board certification through AANP or ANCC
- Minimum of 6 months experience as a Nurse Practitioner preferred
- MSN degree from an accredited nursing program
