Job Description

Advance your clinical skills in a flexible, per diem opportunity focused on home health risk assessments for Medicare Advantage plan members. As a Nurse Practitioner, youll engage with patients in their homes across Morris County, NJ, including areas such as Parsippany-Troy Hills, Mount Olive, and Randolph. These engagementseach lasting about 45 minutesmay be annual health assessments or post-discharge visits aimed at identifying healthcare needs and supporting quality outcomes for older adults.

Schedule:

  • Flexible shifts between Monday and Sunday
  • Design your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Responsibilities:

  • Conduct comprehensive annual health risk assessments within patient homes
  • Review medical history, current health status, and medication usage
  • Educate members on conditions, treatments, and medications
  • Detect care gaps and recommend follow-up or additional interventions as needed
  • Document clinical findings accurately, without the need to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner License in New Jersey
  • Board Certification through AANP or ANCC
  • Master of Science in Nursing (MSN) from an accredited institution
  • Current CPR certification (BLS)
  • At least 6 months of experience as an NP (preferred)
