Advance your career as a Nurse Practitioner specializing in home health care by providing essential risk assessments to Medicare Advantage plan members. This per diem role offers maximum flexibility and the opportunity to make a direct impact in Campbell County, TN, including LaFollette, Jacksboro, and Jellico. As a provider, you will conduct comprehensive in-home visits that support individualized patient wellness.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule
- Set your minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage members
- Evaluate both past and current medical and medication histories
- Deliver patient education on existing health conditions and prescribed medications
- Identify care gaps and offer recommendations for follow-up or improvement
- Note: No treatment planning or prescription responsibilities required
Required Qualifications:
- Active Nurse Practitioner (NP) license in the state of Tennessee
- BLS certification (current CPR)
- Board certification via AANP or ANCC
- Minimum of 6 months of current NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Compensation includes $80-$90 per evaluation, along with stipends for mileage and administrative time.
