This vital role involves working with Medicare Advantage plan members across Campbell County and its communities, including LaFollette, Jacksboro, and Jellico. Schedule:
- Flexible shifts between Monday and Sunday
- Create your own schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform annual home health risk assessments in patient homes
- Evaluate patients prior and current medical histories, existing medical conditions, and medication regimens
- Educate individuals on health conditions and medication usage
- Identify gaps in care and advise on solutions
- No treatment plan creation or medication prescribing required
Desired Qualifications:
- Active Nurse Practitioner license in Tennessee
- CPR certification (BLS)
- Board Certified by AANP or ANCC
- Master of Science in Nursing (MSN) from an accredited school of nursing
- Minimum of 6 months NP experience preferred
