Experience meaningful patient interactions as a Nurse Practitioner specializing in home health, providing comprehensive risk assessments for Medicare Advantage members. This per diem role offers flexibility and autonomy, allowing you to serve communities throughout Williamsburg County, including Kingstree, Hemingway, and Greeleyville.

Schedule:

• Flexible shifts available Monday through Sunday
• Create your own schedule
• Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Complete home-based annual health risk assessments and post-discharge visits for Medicare Advantage beneficiaries
• Evaluate patients prior and current medical histories, chronic and acute conditions, and medication usage
• Offer patient education regarding diagnoses and medication regimens
• Identify care gaps and recommend ways to address them
• No responsibility for treatment planning or prescribing medications

Qualifications:

• Active Nurse Practitioner license in South Carolina
• MSN degree from an accredited school of nursing
• Current Board Certification through AANP or ANCC
• CPR (BLS) certification required
• At least 6 months of experience as a Nurse Practitioner preferred
