This per diem opportunity offers exceptional flexibility, allowing you to create your own schedule to best fit your lifestyle while providing valuable healthcare services to Medicare Advantage plan members across Lafayette and surrounding areas in Tippecanoe County.

Schedule:

• Flexible shifts available between Monday and Sunday
• Set your own hours each week
• Minimum weekly or bi-weekly hourly commitment

Responsibilities:

• Conduct comprehensive in-home risk assessments for Medicare Advantage health plan members
• Review patients prior and current medical histories, existing medical conditions, and medication records
• Educate patients on their diagnoses, chronic conditions, and prescribed medications
• Identify care gaps and provide recommendations for care optimization
• Each visit generally lasts about 45 minutes, with varied naturesuch as annual health assessments or post-discharge visits
• No prescribing of medications or creation of treatment plans required

Qualifications:

• Active Nurse Practitioner license in Indiana
• Board certification through AANP or ANCC
• Current CPR (BLS) certification
• Master of Science in Nursing from an accredited institution
• At least 6 months of current NP experience preferred
