Job Description

Enjoy the flexibility of per diem work as a Nurse Practitioner specializing in home health. This position focuses on conducting comprehensive home health risk assessments within Tippecanoe County, including Lafayette, West Lafayette, and Battle Ground. Visits may take place in different parts of the county based on patient needs, with each risk assessment typically lasting around 45 minutes. These assessments are geared toward Medicare Advantage plan members and may involve annual health reviews or post-discharge evaluations.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own schedule to best fit your availability
  • Minimum weekly or bi-weekly hours required

Key Responsibilities:

  • Complete annual home health risk assessments for Medicare Advantage patients
  • Review prior and current medical history, conditions, and medication usage
  • Educate patients about their medical diagnoses and medication regimens
  • Identify gaps in care and offer recommendations to address them
  • No treatment planning or medication prescribing required

Qualifications:

  • Active Nurse Practitioner license in Indiana
  • Current CPR certification (BLS required)
  • Board Certification through AANP or ANCC
  • Minimum of six months NP experience preferred
  • MSN from an accredited nursing school
