Step into a vital role focused on improving patient care for Medicare Advantage plan members throughout Thurston County. In this per diem position, you'll conduct home health risk assessments across Lacey, Olympia, and Tumwater. These visits, averaging 45 minutes each, are essential in identifying patient needs and ensuring the highest standards of carewhether completing an annual health assessment or a post-discharge check-in.

Schedule:

• Flexible shifts available seven days a week
• Create your own schedule to suit your availability
• Set your preferred minimum weekly or bi-weekly commitment

Key Responsibilities:

• Perform thorough home health risk assessments for Medicare Advantage health plan members
• Review and document prior and current medical history, existing medical conditions, and medication regimens
• Educate patients on their health conditions and the effective management of prescribed medications
• Identify and communicate gaps in care, offering practical recommendations for patient well-being
• Note: No prescribing or creation of treatment plans required

Qualifications:

• Current Nurse Practitioner licensure in Washington State (WA)
• Board Certification from AANP or ANCC
• Active CPR certification (BLS)
• Minimum of 6 months recent experience as a Nurse Practitioner is preferred
• Master of Science in Nursing (MSN) from an accredited institution
