Seeking a dedicated nurse practitioner to perform home health risk assessments across Glades County, FL. This per diem opportunity is designed for clinicians who value flexible scheduling and direct patient impact. The role involves visiting Medicare Advantage members in their homes for focused clinical evaluations, educational discussions, and care recommendations.
Schedule:
- Flexible shifts available Monday through Sunday
- Self-scheduled shifts to accommodate your availability
- Minimum weekly or bi-weekly commitment of hours required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members
- Review and evaluate patient medical history, conditions, and medication use
- Educate patients on their medical diagnoses and medication regimens
- Identify care gaps and recommend appropriate next steps or resources
- No responsibilities for prescribing medications or developing treatment plans
Qualifications:
- Active Nurse Practitioner license in the state of Florida
- Current BLS CPR certification
- Board certified by AANP or ANCC
- At least six months of current experience as a Nurse Practitioner preferred
- MSN from an accredited nursing program
This position offers a compensation range of $80-$90 per completed evaluation, along with a stipend for mileage and administrative duties, ensuring your time and travel are recognized and rewarded.
