Experienced Nurse Practitioners are needed for per diem opportunities conducting home health risk assessments in Adair County, Missouriincluding Kirksville, Greentop, and Novinger. This role offers flexibility in scheduling and allows you to make a direct, positive impact on the health of Medicare Advantage plan members during annual health assessments or post-discharge visits.
Schedule:
- Flexible shifts available any day, Monday through Sunday
- Create your own schedule to balance work and life
- Minimum commitment on a weekly or bi-weekly basis
Key Responsibilities:
- Perform in-person home health risk assessments (average 45 minutes per visit) for Medicare Advantage members
- Assess medical history, current health status, and medication use
- Deliver clear, compassionate education to patients regarding medical conditions and medications
- Identify gaps in care, documenting findings and offering resolution recommendations
- No requirements to create treatment plans, prescribe medications, or follow up
Qualifications:
- Active Nurse Practitioner license in Missouri
- MSN degree from a nationally accredited nursing school
- Current CPR certification (BLS)
- Board Certification via AANP or ANCC
- At least 6 months of Nurse Practitioner experience preferred
You will receive $80$90 per completed evaluation, along with a mileage stipend and additional compensation for administrative duties. This per diem position provides autonomy to manage your caseload and the satisfaction of supporting members well-being right in their homes.
