Job Description
Deliver comprehensive and impactful healthcare in a per diem Nurse Practitioner role focused on home health risk assessments. In this position, you will provide crucial evaluations for Medicare Advantage plan members, conducting thorough risk assessments in private residences throughout Otero County, including the La Junta, Rocky Ford, and Fowler areas. Each visit, lasting approximately 45 minutes, may vary in natureranging from annual assessments to post-discharge follow-upsensuring that every appointment brings new opportunities to make a meaningful difference in patients lives.
Schedule:
- Flexible shifts available between Monday and Sunday
- Set your own schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual risk assessments for Medicare Advantage members in home settings
- Review prior and current medical histories, chronic conditions, and medication usage
- Educate patients on their health conditions and prescribed medications
- Identify care gaps and recommend resolutions
- No need to develop treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Colorado
- Current CPR (BLS) certification
- Board Certification (AANP or ANCC)
- At least 6 months of Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
