Job Description

Seeking a skilled Nurse Practitioner to deliver home health risk assessments on a per diem basis in Alamosa County. This position is tailored for professionals who prefer autonomy, flexibility, and a patient-centered approach. As part of this role, you will travel to the homes of Medicare Advantage members in communities such as Alamosa, Blanca, and Hooper to complete comprehensive risk assessments.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Build your own schedule to fit your lifestyle
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage plan members
  • Review and assess prior/current medical history, diagnoses, and medication usage
  • Educate members about their medical conditions and medication regimens
  • Identify care gaps and recommend steps to address them
  • Visits include annual assessments and post-discharge evaluations as needed
  • No prescription of medications or development of treatment plans required

Qualifications:

  • Valid Nurse Practitioner license in Colorado
  • Current Basic Life Support (CPR) certification
  • Board certification from AANP or ANCC
  • 6 months or more of recent nurse practitioner experience preferred
  • Master of Science in Nursing from an accredited institution
