Job Description

Step into a rewarding per diem opportunity conducting in-home risk assessments throughout Campbell County, TN. As a skilled Nurse Practitioner, youll provide critical risk assessments for Medicare Advantage plan members, helping to improve care outcomes and patient understanding. Each visit typically takes about 45 minutes, ranging from annual health evaluations to post-discharge assessments. Shifts are available in LaFollette, Jacksboro, Jellico, and surrounding locations based on assignment.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Opportunity to tailor your own schedule
  • Minimum hourly commitment required on a weekly or bi-weekly basis

Responsibilities:

  • Complete comprehensive home health risk assessments for Medicare Advantage members
  • Review and evaluate patients prior/current medical history, medical conditions, and medication use
  • Deliver clear education regarding ongoing conditions and medication regimens
  • Identify care gaps and propose actionable recommendations
  • No responsibility for writing treatment plans or prescribing medications

Qualifications:

  • Active Nurse Practitioner license valid in Tennessee
  • Current BLS (CPR) certification required
  • Board Certified through AANP or ANCC
  • Minimum of 6 months of recent Nurse Practitioner experience preferred
  • MSN degree from an accredited nursing program
