Job Description
Make a meaningful impact in patients lives by performing in-home health risk assessments for Medicare Advantage members across Campbell County, TN. As a per diem Nurse Practitioner specializing in home health, you will work autonomously in areas including LaFollette, Jacksboro, and Jellico, providing vital evaluations and connecting patients to the care resources they need.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose a minimum weekly or bi-weekly hourly commitment to match your lifestyle
Key Responsibilities:
- Complete annual or post-discharge health risk assessments for Medicare Advantage plan participants in their homes
- Review and document prior/current medical history, existing health conditions, and medication regimens
- Educate patients on diagnoses, ongoing medical issues, and medication usage to empower better personal health management
- Detect gaps in care and recommend appropriate follow-up, ensuring continuity and quality of care
- No treatment plans or prescription responsibilitiesfocus on assessment and education
Qualifications:
- Active Nurse Practitioner license in Tennessee
- Board Certified (AANP or ANCC)
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience is preferred
Your commitment will be rewarded with compensation of $80-$90 per completed evaluation, plus a mileage stipend and administrative time, ensuring you are supported while working throughout Campbell County.
