Deliver high-quality patient care as a Nurse Practitioner specializing in home health risk assessments on a per diem basis. This role involves visiting Medicare Advantage plan members throughout Pittsburg County for scheduled assessments, with flexible shifts and the opportunity to design your own schedule.
Schedule:
- Flexible hours available Monday through Sunday
- Self-scheduling to best suit your preferences
- Options for minimum weekly or bi-weekly commitment
What Youll Do:
- Conduct comprehensive home health risk assessments for Medicare Advantage members in various locations within Pittsburg County
- Perform detailed evaluations of current and previous medical histories, chronic conditions, and medication regimens
- Educate patients on existing medical conditions and prescribed medications
- Identify care gaps and provide recommendations for follow up or additional care
- Focus exclusively on assessmentsno requirement to develop treatment plans or prescribe medications
Required Qualifications:
- Active Nurse Practitioner License in Oklahoma
- Board Certification through AANP or ANCC
- Current BLS/CPR certification
- MSN degree from an accredited nursing program
- At least 6 months of recent experience as a Nurse Practitioner preferred
