Job Description

Make a meaningful impact by providing essential care in a dynamic home health setting. This role offers the opportunity to perform home health risk assessments for Medicare Advantage plan members throughout Idaho County, ensuring personalized and patient-focused care in communities such as Grangeville, Cottonwood, Kamiah, and surrounding areas.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Freedom to create your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct comprehensive home health risk assessments, each averaging 45 minutes per visit
  • Review and evaluate patients prior and current medical history, conditions, and all medication usage
  • Deliver clear, compassionate patient education regarding medical concerns and medication management
  • Identify any gaps in patient care, providing concise recommendations for follow-up or resolution
  • Focus solely on evaluationsno requirement for treatment planning or prescribing medications

Qualifications:

  • Active Nurse Practitioner license in Idaho (ID)
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited program
  • At least 6 months of current NP experience preferred

This per diem position offers a highly competitive compensation structure of $80 to $90 per evaluation, with additional mileage and administrative stipends, allowing you to maximize your earnings and maintain autonomy over your schedule.
