An opportunity is available for an experienced Nurse Practitioner to perform home health risk assessments on a per diem basis for Medicare Advantage plan members throughout Howard County, IN. This role offers the ability to create your own schedule and work flexible shifts, supporting quality care in a home setting in locations such as Kokomo, Greentown, and Russiaville.
Schedule:
- Flexible shifts between Monday and Sunday
- Choose your own working days
- Set your minimum weekly or bi-weekly hours
Key Responsibilities:
- Complete annual health risk assessments for home-based Medicare Advantage members
- Review patients prior and current medical history and medication lists
- Perform health evaluations, including post-discharge assessments if required
- Educate patients on their medical conditions and medications
- Identify care gaps and suggest interventions to resolve them
- No requirement to establish treatment plans or prescribe medications
Qualifications:
- Active Nurse Practitioner license in Indiana
- Current CPR certification (BLS)
- Board certification via AANP or ANCC
- Preferred: minimum 6 months of experience as a Nurse Practitioner
- MSN from an accredited nursing program
This per evaluation position offers a competitive rate of $80-$90 per completed assessment, in addition to a mileage stipend and compensation for administrative time.
