Job Description

Step into a dynamic per diem role as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage members. This opportunity offers flexibility and autonomy, allowing you to tailor your own schedule with shifts available Monday through Sunday and a minimum weekly or bi-weekly hourly commitment. Youll conduct each visit within Howard County, serving residents in locations including Kokomo, Greentown, and Russiaville.

Responsibilities:

  • Perform annual and post-discharge home health risk assessments, with each evaluation taking approximately 45 minutes
  • Review and evaluate prior/current medical history, chronic conditions, and medication regimens
  • Identify care gaps and make recommendations to help close them
  • Offer patient education regarding ongoing conditions and medications
  • Collaborate to ensure continuity of care, but no treatment planning or prescribing necessary

Qualifications:

  • Active Nurse Practitioner License in Indiana
  • Board Certified through AANP or ANCC
  • MSN from an accredited nursing program
  • Current CPR certification (BLS)
  • At least 6 months of nurse practitioner experience is highly preferred
  • Experience in home health or risk assessment is a plus
